Admissions Process for Madeley Academy Sixth Form

Application forms are available to download from our website, either on the main "Post-16" page, in the "Documents to Download" section, or alternatively there is a link to the form below.

The subject preferences you indicate on the application form will help us to ensure we have the right course options available for you.

We will reply to you when we can confirm if we can provide the courses you have requested.

We may invite you into the Academy to discuss your chosen subjects.  Internal and external applicants will be invited for interview with a senior member of staff to discuss choices when examination results are confirmed.
